Alopecia Areata: Aetiology and Pathogenesis

Abstract
Alopecia areata is a common chronic inflammatory disorder of the hair and nails.
Genetic factors are important in determining susceptibility and disease severity but it is
unlikely that alopecia areata is caused by a single gene defect. In common with other
inflammatory diseases, environmental factors may be involved in initiating disease
expression.

Research into the pathogenesis of alopecia areata has been concentrated in two main areas - the role of the immune system and the nature of the hair follicle pathology - but our understanding of the relationship between these is limited. In this article, the aetiology and pathogenesis of alopecia areata is reviewed. Some promising lines of research are now in progress and the future challenge is to formulate hypotheses on the pathogenesis which encompass the diversity of information now available from the various clinical and experimental studies.
